Date: -------------------------------------------------------------------------------- ================================================================================ Death Notes (Husband) ================================================================================ According to obit on p B5, 4 Sep 1951 of Columbus Dispatch, Ernest was "[. . .] killed Friday in Rock Island Terminal, Des Moines, Iowa [. . .]." -------------------------------------------------------------------------------- Death certificate from Iowa, yr 1951, Birth [stamp] No. 18578, State File No. 77C-51-1526 confirms his place and time of death. His body was removed from Iowa to Ohio on Sept 1, 1951 for burial at Union Cemetery, Columbus, Ohio. ================================================================================ Medical Notes (Husband) ================================================================================ per death certificate issued by Iowa Dept of Health, he was crushed between a small truck and the dock at 1pm on 31 Aug 1951 at the Freight Depot, where he was employed. It seems he may've been employed under the name Ben F Moore, as this was listed as his "alias." The MD reported that he called time of death at 1:15pm, only 15 minutes after the supposed time of accident. The informant appears to <possibly>be Edward E Slagle (Ernest's son), but the signature is VERY difficult to make out as it is fairly illegible. Thus, this is merely speculation about the son being informant. However, the informant claims that Ernest's father was Andrew Slagle (which was already known at the time that the certificate was purchased) and that the mother was Rebecca Hart (her maiden name was previously unknown). Aditionally, the certificate lists his POB to be Hocking County, though, which it was previously believed his POB to be Pretty Run in Eagle Twp, Vinton County, Ohio. (27) ================================================================================ Death Notes (Wife) ================================================================================ Everett Marchington was her informant for death record. He listed her mother as Emma Cockerell (not Cottrill) and father as Wm Dixon. Although these are a bit off, they are close enough considering Everett had not known her all her life, and in fact, had not met her father as he was already deceased upon the Marchington's marriage. The stillbirth record of Faye's baby girl, signed and informed by her mother and father (Cashes and Clara Emma Cottrill Dixon), proves beyond a shadow that these were the same people. ================================================================================ Burial Notes (Wife) ================================================================================ buried near ex-husband Earnest Slagle, section 71 lot 71. ================================================================================ Medical Notes (Wife) ================================================================================ Time of death, 8:30 pm on 2 Oct 1948 at St Anthony's Hosp, where she had only been admitted 1 hr prior to death.
